Dr Ashandren Naicker is a specialist cardiologist who treats heart arrhythmia, murmurs and cardiac disease from Mount Edgecombe and Entabeni Hospital based in Durban. Dr Naicker attained his Medical Degree from the University of KwaZulu-Natal. He completed specialist training as a physician and has a Fellowship in internal medicine. After Dr Naicker completed his specialisation in cardiology, he contributed to a study on advanced aortic stenosis which appeared in the SA Heart Journal. To add to his list of accolades, he has his Masters in Medicine and specialises in cardiac care, interventional cardiology, heart disease and echocardiogram.

  • Heart arrhythmia
    Heart arrhythmia occurs when electrical impulses that regulate the heartbeat are out of sync. Because electrical impulses are irregular, the heart beats too fast or too slow. The heart starts to race and can become fatal, especially if the muscle is already weak. There are two types of heart arrhythmia, tachycardia and bradycardia. Tachycardia refers to a fast heartbeat, and bradycardia is a slow heartbeat. Atrial fibrillation and flutters are tachycardias that occur as a result of uncontrollable electrical impulses within the atria. If not treated, atrial fibrillation can cause a stroke.
  • Congestive heart failure
    Congestive heart failure happens when the heart fails to pump blood adequately. Coronary artery disease arises when the blood vessels within the heart constrict. As soon as the arteries narrow, the heart muscle weakens. Hypertension and coronary artery disease stiffen the heart muscle and affects its ability to pump blood as it should. Fatigue, a rapid heartbeat, abdominal swelling, shortness of breath, nausea and dizziness are symptoms of congestive heart failure. If not resolved early enough, the condition can lead to heart failure. Signs of heart failure include chest pain, fatigue and an irregular heartbeat.
  • Coronary artery disease
    Coronary arteries are blood vessels that transport blood to the heart. Coronary artery disease occurs when the blood vessels start to narrow or become blocked. This particular disease begins when one is young and builds up later in life. Typically, the coronary arteries are elastic and lined with layers of cells known as the endothelium. These cells safeguard the inner lining of the vessel walls, but with age, steaks of plaque tend to stick to these walls. Atherosclerosis is the accumulation of plaque and cholesterol, which restricts blood supply to the heart muscle. When the heart doesn't receive an adequate supply of blood, nutrients and oxygen, it fails to work correctly. A weakened heart results in angina or chest pain.
  • Atrial fibrillation
    Atrial fibrillation is a form of tachycardia which results in an irregular, rapid heartbeat. This condition occurs when the top chambers of the heart work irregularly and inconsistently. The atria and ventricles remain out of sync. Atrial fibrillation symptoms include fatigue, difficulty breathing and heart palpitations. Also, blood clots may form in the top chambers of the heart and spread to other organs which result in ischemia or reduced blood flow. Dr Naicker prescribes beta-blockers and calcium channel blockers to stabilise heart rate and relax atrial muscles.
  • Valvular heart disease
    Valvular heart disease arises when one of the heart’s valves sustains damage due to disease. Healthy valves regulate blood flow to and from the heart. A diseased valve cannot open and close properly, and because of this, blood flow is disrupted. Leaky valves result in regurgitation when the blood backs up into its original chamber. Because of this, insufficient blood flows back to the heart. Another issue is stenosis. Stenosis occurs when the entrance of the valve becomes narrow and stiff, and as a result, the valve cannot open completely. Dr Naicker prescribes calcium-channel blockers, beta-blockers and diuretics to minimise fluid retention.
  • Cardiomyopathy
    Cardiomyopathy is a form of heart disease that makes it difficult for the heart to regulate blood to other parts of the body. There are three types of cardiomyopathy which include dilated cardiomyopathy, hypertrophic cardiomyopathy and restrictive cardiomyopathy. Dilated cardiomyopathy occurs when the left ventricle dilates and therefore fails to pump blood from the heart. Hypertrophic cardiomyopathy happens when heart muscle thickens and affects the left ventricle. Restrictive cardiomyopathy occurs when the heart loses its elasticity and fails to expand between heartbeats.
  • Hypertension
    Pulmonary hypertension is high blood pressure that affects the arteries within the lungs and the right side of the heart. The blood vessels within the lungs become blocked or start to narrow. Damage to the lungs causes blood pressure in the arteries of the lungs to rise. The heart, therefore, works harder to deliver blood to the lungs. Because the heart works harder than what's used to, over time, it weakens and may give out. Dr Naicker prescribes medication or performs surgery to resolve pulmonary hypertension.
  • Chest pain
    Chest pain is also known as angina and occurs when the heart doesn't receive an adequate supply of oxygenated blood. Angina is a sign of an underlying cardiac disease such as coronary artery disease. Different types of chest pain include stable angina, unstable angina, Prinzmetal angina and microvascular angina. Angina pectoris is a form of chest pain due to coronary artery disease and occurs when the heart doesn't receive enough blood as it should. Unstable angina is a coronary syndrome that occurs unexpectedly due to the build-up of fat in the coronary arteries. Prinzmetal’s is a typical form of chest pain that occurs as a result of emotional stress. Microvascular angina occurs as a result of cardiac disease that targets the smallest coronary artery blood vessels.
  • Heart palpitations
    A heart palpitation is the pounding sensation of the heart. Exercise, medication, stress and medical conditions trigger heart palpitations. Particular heart palpitations that don't require a further evaluation are those that are infrequent and are over within a matter of seconds. Heart palpitations are an indication of heart arrhythmia and may also be a sign of hyperthyroidism. Dr Naicker first needs to investigate the cause of heart palpitations and detect an underlying illness that may contribute to the condition.
  • Dizziness and Blackouts (syncope)
    Syncope is a sudden blackout that occurs due to reduced blood flow to the brain. Vasovagal syncope is common and happens when there's a sudden drop of blood flow to the brain due to low blood pressure. Situational syncope is triggered by emotional stress, dehydration, anxiety, fear and hyperventilation. Dr Naicker resolves syncope through the use of medication, surgery to treat valve disease and catheter ablation for speedy arrhythmias.
